Do you have a barn?
Thank you for considering adopting a rescue animal and helping us save lives. We look forward to working with you to find the perfect match. One of our adoption counselors will be contacting you back within 2 business days. If you do not get a response please email us at firstname.lastname@example.org to confirm receipt of your application. To expedite the processing of your application please fill it out completely and honestly. Once you click the submit button a confirmation page will appear. If you do not see the confirmation page then the application did not go through. We are an all volunteer organization. We appreciate your patience.
For the health and socialization, we advocate adopting kittens 4 months of age and under in pairs. Our adoption fee is $100 for cats under 5 years old, $50 for cats 5 years and older and $150 for dogs and will help save the lives of more animals. All cats and dogs are vaccinated, tested for FIV/Felv and spay/neutered before adoption. Adoption is a life choice so we know you will be understanding of adoption fees not being refundable.

List all members of your household and their ages. Include family, roommates, and frequent visitors:
Please list all of the pets currently living in your house. Then list any other pets you've had as an adult, and their current status (i.e. deceased, living with friends, lost, etc...). For any deceased pets please list cause of death if known.
Do you own or rent? If you rent, list your landlord's address and phone number.
Current or previous veterinarian's name, hospital, city, and PHONE NUMBER. Please call your vet's office and give permission for them to give us a medical history on your pets.
If you do not have a vet, please list 3 personal references (name, phone number and email).
Pets often live 15 plus years. Are you willing to make the lifelong commitment?
